Cancellation Policy
You will receive an Activation Date with your confirmation email Renewal date 28 days from Activation Date NON REFUNDABLE Additional Hours billed @ 120% x Hourly Rate. Price Per Session/4Hours= Hourly Rate. Payment of Additional Hours due 48hours prior to monthly renewal date. *ADLs or Activities of Daily Living are not included with this service and additional charges will apply with service is rendered. ** Transportation to Medical Appointments must be from a 3rd party. Consent to Medical Information may be required. ***Transportation Vehicle not provided, drivers will have driver's insurance and valid Drivers LIcense.
